Bio-Identical Hormone Replacement Therapy

 

Bio-Identical Hormone Replacement Therapy  - or BHRT -  is a topic of great interest to women.  Many have heard about it from Suzanne Somers and are thinking, “Wow!  She looks pretty good!  Maybe BHRT is the way to go!”   

So what is BHRT?   Bioidentical hormone replacement therapy (BHRT) is the use of supplemental doses of  hormones with a chemical structure identical to hormones naturally produced in the human body. BHRT differs from conventional hormone replacement therapy, which by definition uses animal or synthetic hormones whose structures are similar or identical to those produced in the human body. What animal hormones are they referring to?  Take Premarin for example:
Pre = pregnant       Mar = mare (female horse)          Rin = urine. 

Premarin is made from pregnant mare urine.  My question to you is, just how many pregnant horse hormones does a human female really need?


BHRT is not simply a magic wand.  Hormones are extremely potent and supplementation must be undertaken with great care and appreciation for their impact.  With this in mind, I approach hormone replacement with both eyes open.  I do an extensive interview on the first visit to evaluate hormone symptoms, followed by saliva testing to determine hormone levels.  Some ask “why do the testing if the symptoms are evident?”  The reason for this is that symptoms of estrogen excess are the same as estrogen deficiency – and the same goes for progesterone, so testing of hormone levels is a necessity for proper care. 

When considering the dosage to prescribe, I evaluate what levels of supplementation the patient has already received, factor in the presenting symptoms and consider the lab results.  Based on this I make my recommendations.  I begin with a low dose and ask the patient to keep in touch so we can adjust the dose as needed.  I want to know that the therapy is working and the patient’s needs have been met by the prescribed dosage.  Follow up labs with saliva, urine or blood are done after 3 – 4 months to ascertain therapeutic levels.
